UHC Community Plan of Florida participates in the Statewide Medicaid Managed Care (SMMC) Managed Medical Assistance (MMA) program. Providers must first enroll as a Florida Medicaid provider through AHCA, then apply to UHC via CAQH ProView. Active Florida medical license and NPI are required. Service areas are organized by AHCA regions. The plan also participates in the Long-Term Care (LTC) managed care program in select regions.
