Fully licensed LCPCs and LMFTs have been individually enrollable in Illinois Medicaid since December 2021, and you can still find directories, and consultants, saying they are not, because the regulation that lists independent practitioners was never amended. Meanwhile the commercial side has its own identity trap: Cigna members' behavioral care runs through Evernorth, a separate entity with its own intake. An Illinois BH practice deciding where its clinicians can earn needs both facts straight.

Common mistake

Cigna members, Evernorth network. Apply to the right entity.

Behavioral health for Cigna members runs through Evernorth Behavioral Health, renamed from Cigna Behavioral Health in 2021 and operated as a separate network. BH clinicians contact Evernorth at 1-800-926-2273 rather than Cigna medical credentialing, and an application filed through the medical flow is routed to the wrong entity without a notice. Evernorth reads your CAQH ProView profile only with permission, so authorize it explicitly before applying, or the electronic pull fails silently.

Hard rule

The regulation was never updated. The coverage is real anyway.

Illinois implemented LCPC and LMFT Medicaid coverage through statute, IMPACT enrollment configuration and provider notices, and never amended 89 IAC 140.452, the regulation listing independent practitioners. Read the regulation alone and you get a pre-2021 answer: only clinical psychologists, LCSWs and psychiatrists. The HFS notice is unambiguous, coverage of services by enrolled LCPCs and LMFTs began with dates of service on or after December 1, 2021, IMPACT provider type Behavior Health Services, on the LPHA fee schedule. If someone tells you an LCPC cannot enroll in Illinois Medicaid, they are reading the regulation, not the law.

Illinois is unusually permissive about who may render Medicaid behavioral health and unusually strict about how it is billed. The pre-licensed tier works only through enrolled facilities. Full licensees have individual lanes on both the Medicaid and commercial sides.

How to apply to Evernorth from Illinois

Illinois adds two scheduling realities to the national process: a slow licensing regulator and no compact shortcut.

  1. 1 Plan the licence first. IDFPR budgets 10 to 12 weeks for physicians and behavioral boards run their own queues. Illinois has not enacted the Counseling Compact, the bill failed in January 2025, so out-of-state LPCs plan full endorsement.
  2. 2 Get CAQH ProView complete, attested, and authorized to Evernorth specifically.
  3. 3 Apply to Evernorth at 1-800-926-2273, not through Cigna medical credentialing.
  4. 4 Expect 45 to 60 days of credentialing by Cigna's published statement, with recredentialing every three years.
  5. 5 If Medicaid volume is in the plan, run the IMPACT enrollment in parallel: individual enrollment for LCPCs and LMFTs, facility lanes for the pre-licensed tier.

Worth knowing

Screen Illinois rosters against the live sanctions portal, not the frozen files

Illinois's Medicaid sanctions list moved to an HFS OIG portal, and the old direct-download .xls files on illinois.gov are frozen 2021 and 2022 snapshots that still look plausible. A roster screened against the frozen files misses years of actions. The live list held over three thousand rows at last pull, and every action type on it, Terminated, Barred, Voluntary Withdrawal, Exclusion, Suspended, is exclusionary. For a group hiring into Illinois Medicaid work, this screen belongs in onboarding, next to the federal OIG check.

The Illinois sequencing problem is the licence, not the payer

Evernorth's 45-to-60-day window is ordinary. What stretches Illinois timelines is everything before it: a licensing regulator that budgets months, no compact relief for counselors, and the $5 Illinois Controlled Substance License that prescribers forget and then cannot use their DEA without. A practice recruiting an out-of-state clinician into Illinois should count backwards from the target start date through Evernorth's window, then CAQH preparation, then the IDFPR queue, and let that arithmetic set the offer date. The payers are not the slow part here. The state is.

Evernorth Behavioral Health — formerly Cigna Behavioral Health (renamed September 1, 2021) — is the behavioral network for Cigna Healthcare members. IMPORTANT: as of June 1, 2026, Evernorth has paused accepting applications from new individual and clinic behavioral providers; affected clinicians should complete the Provider Interest Form to be notified when applications reopen (revisit after September 1, 2026). Facility applications are still accepted, and applications started before June 1, 2026 are unaffected. When open, providers apply via the Provider Information Form, then complete and attest a CAQH ProView application. Requirements include an unrestricted state license, NPI, current DEA certificate where applicable, board certification if applicable, and professional liability insurance (typically $1M per occurrence / $3M aggregate unless the state mandates lower).
