What the map reveals

One company runs over half the country

Gainwell Technologies operates enrollment systems in 27 states, but they are different platform products with different screens, so experience in one state rarely transfers to another.

Two states literally share one system

Illinois runs its IMPACT enrollment on Michigan's cloud MMIS through an interstate alliance. One platform, two states, since 2015. Nothing else in Medicaid works this way.

Ohio split enrollment from claims

Maximus runs Ohio's PNM portal for enrollment and centralized credentialing while Gainwell only pays claims. Enrollment questions go to a different company than payment questions.

Three programs are publicly operated

California (DHCS), Minnesota (DHS), and South Carolina (Clemson University) run their own systems with no commercial fiscal agent for enrollment.

Contracts are moving right now

DC moved to Gainwell in March 2026, Virginia moved enrollment to Gainwell, Maryland's MPRIME replacement lands in October 2026, and South Carolina has a replacement in procurement. Guides written last year may already be wrong.

South Dakota and Maryland do not publicly name their current operating vendor. We mark them honestly instead of guessing, and update this page when official sources settle it.

Every entry was checked against an official source in July 2026: state Medicaid agency pages, contractor announcements, and the CMS Medicaid Enterprise Systems contract status reporting. 48 of 51 programs are confirmed from an official source, one is inferred from contract lineage and labeled as such, and two are honestly marked unknown. Each state contracts its own operator. Gainwell Technologies runs enrollment systems in 27 states, Acentra Health in 7, Conduent in 4, and Maximus in 3, while California, Minnesota, and South Carolina operate their own. Find your state in the table above for the portal and the company behind it.

Is Medicaid provider enrollment the same in every state?+

No. Every state runs its own program, portal, document requirements, and timelines. Even states served by the same contractor usually run different platform products, so instructions from one state rarely transfer to another.

What is an MMIS or fiscal agent?+

The Medicaid Management Information System (MMIS) is the platform a state uses to run Medicaid operations, including provider enrollment and claims payment. The fiscal agent is the contractor the state hires to operate it. On this page, "operated by" names that contractor.

Do any states share a Medicaid enrollment system?+

Yes, in exactly one case: Michigan and Illinois share one cloud MMIS through the IMPACT interstate alliance. Two more pairs run the same product on separate systems: Arizona and Hawaii on the jointly developed evoBrix platform, and New Hampshire and North Dakota on Conduent's Health Enterprise.
