VSP Vision Care: VSP Choice network. Largest US vision network with 91,000+ providers. Apply via VSP provider portal (vsp.com). Must have active optometry or ophthalmology license. VSP handles credentialing internally (not CAQH). Processing 60-90 days. State licensing: Illinois Department of Financial and Professional Regulation (Optometry) issues OD licenses. Therapeutic Pharmaceutical Agents (TPA) certification available. Ophthalmologists (MD/DO) also accepted via state medical board license. VSP is member-governed. Optical lab information required during application.
