CAQH ProView profile, required in most states, submitted through the Provider Express join flow. Group contracts are limited and require at least 5 credentialed providers.

Behavioral health routing: Optum Behavioral is UnitedHealthcare's behavioral network. All UHC behavioral credentialing lives here, separate from the medical network.

The stall to avoid: Filing a behavioral application in UHC's Onboard Pro instead of Provider Express is the number one stall: wrong queue, no error message, no progress. Solo clinicians who hit the network need gate should document the access gap they fill, including specialty, language, and geography.

From PayerReady's payer enrollment research, built from the payer's own provider manuals and enrollment pages. Payer processes change; when a fact is not published by the payer, we say so rather than guess.

District Of Columbia-Specific Requirements

Optum Behavioral Health (United Behavioral Health) credentials behavioral and mental health clinicians through the Provider Express portal — register or log in and use the "Join Our Network" feature to start the application. CAQH participation is required for credentialing in most states; providers must authorize United Behavioral Health to access their CAQH profile. Core documents: a signed W-9, current professional liability insurance, and current state license(s). As of October 1, 2025, Optum uses Medallion for primary source verification (Verisys handles Medicaid providers in Arizona, Nebraska, and Texas). Recredentialing occurs every 36 months. Optum does not publish a committee-decision timeframe; track status in Provider Express.
