Cigna in Alabama: one of the few states where the payer map is actually simple, except for one door

Alabama never adopted risk-based Medicaid managed care, so there is no Cigna Medicaid plan here and no MCO panel to chase: Medicaid is fee-for-service with the state. That makes Cigna a purely commercial decision, which should be simple. The complication is that behavioral health is not Cigna at all, it is Evernorth, a separate entity with its own intake, and applications addressed to the wrong one stall silently.

Common mistake

Behavioral health is Evernorth, not Cigna

The one complication in Alabama's otherwise simple map: a therapist or psychiatrist joining "Cigna" is actually joining Evernorth Behavioral Health, the separately-operated BH network that took over the Cigna Behavioral name in 2021. Its intake is its own phone line, 1-800-926-2273, and its credentialing runs on CAQH. Send the packet down Cigna's medical path instead and nothing rejects it, nothing processes it, and nobody calls. Weeks of silence on a BH file is the signature of this exact mistake.

Hard rule

Cigna only reads your CAQH profile if you said it could

Cigna's process is explicit: if your application already exists on CAQH, then with your permission Cigna accesses it electronically. Without that authorization the pull fails, and it fails silently. Set CAQH to global authorization or add Cigna and Evernorth specifically before applying. The other quiet application killer is a W-9 whose practice address does not match the tax ID on file, which bounces the contract at the finish line.

Commercial Cigna vs Alabama Medicaid: two different worlds, no overlap

In most states the same clinician chases a commercial panel and two or three Medicaid MCO panels. Alabama removes the second half entirely, and replaces it with a state fee-for-service system that has its own gates.

Cigna commercial Alabama Medicaid
Managed-care panel to join Yes, via CignaforHCP or Evernorth None. No capitated MCOs exist, billing is FFS with the state
What ACHN is Not applicable A care-coordination network, its own page says delivery of medical services is not part of the program
BH delivery Evernorth Behavioral Health, separate entity ADMH-certified agencies, plus licensed practitioners under the EPSDT-referral framework
Associate-licensed clinicians No published lane ALC bills own NPI at 75%, but only for under-21 EPSDT-referred recipients

How to apply to Cigna from Alabama

The application is national. The Alabama part is knowing what not to look for.

  1. 1 Medical clinicians: apply through the CignaforHCP portal.
  2. 2 Behavioral health clinicians: contact Evernorth at 1-800-926-2273. It is a separate network with its own credentialing.
  3. 3 Authorize Cigna, and Evernorth if applicable, in CAQH ProView before you apply, and confirm the W-9 matches the tax ID and practice address exactly.
  4. 4 Expect credentialing to run 45 to 60 days by Cigna's own statement, then recredentialing every three years.
  5. 5 Do not budget time for an Alabama Cigna Medicaid panel. It does not exist. If Medicaid revenue is in the plan, that is a separate FFS enrollment with the Alabama Medicaid Agency.

Worth knowing

Read the scope line before staffing associates toward Alabama Medicaid

Alabama's manual gives associate counselors a genuinely rare thing, direct enrollment billing their own NPI, and then fences it: licensed counselors and marriage and family therapists are enrolled only for services to recipients under 21 referred from an EPSDT screening, at 75% of the allowable amount. It is a real lane for a child-and-adolescent practice and a mirage for an adult outpatient one. Adult Medicaid work in Alabama reaches psychologists and licensed social workers, and rehab-option services require the agency to hold Department of Mental Health certification first.

Counselors eyeing Alabama: the compact is enacted here, not live

Alabama passed the Counseling Compact early, in 2022, but as of mid-2026 it is not yet issuing privileges, so an out-of-state LPC still plans a full endorsement through the Alabama Board of Examiners in Counseling before any payer application can start. Sequence accordingly: licence first, then Cigna or Evernorth with CAQH authorized, then the 45-to-60-day credentialing run. And a closing discipline that matters more in Alabama than most places, because no state credentialing statute has been identified here to lean on: keep dated proof of the complete application. The payer's own published timeline is the only clock you have, and holding them to it starts with proving when it started.

Alabama-Specific Requirements

Cigna Healthcare credentials medical providers through CAQH (the Universal Credentialing DataSource). To begin, call Cigna Healthcare Provider Services at 1-800-882-4462 and choose the credentialing option; complete or authorize your application at CAQH.org so Cigna can pull your verified data electronically. Credentialing typically takes 45 to 60 days once Cigna has your completed materials. Required credentials include an unrestricted state license, valid unrestricted DEA certificate, board certification status, malpractice coverage, and a five-year work history. Recredentialing is required every three years in most states; Cigna emails you to re-attest your CAQH profile and sends up to three reminders before network termination. Check status by emailing PSSCentral@Cigna.com with your name and TIN. Cigna's behavioral health network is operated separately by Evernorth Behavioral Health.
