Nevada-Specific Requirements

CHAMPVA (the Civilian Health and Medical Program of the Department of Veterans Affairs) works differently from a managed-care network: there is NO credentialing, contracting, or network-join process. Per the VA, "CHAMPVA does not have contract providers." Any provider who is properly licensed in their state, is not on the Medicare exclusion list, and agrees to accept the beneficiary can treat and bill CHAMPVA. The one setup step for payment is enrolling in Electronic Funds Transfer (EFT). Bill electronically using payer ID 84146 (medical) or 84147 (dental), or mail paper claims to VHA Office of Integrated Veteran Care, CHAMPVA Claims, PO Box 30750, Tampa, FL 33630-3750. Claims must be filed within 1 year of the date of service, and providers who accept a beneficiary cannot balance-bill. CHAMPVA is a uniform national program — the rules are identical in every state.
