Medical and surgical supplies. Most A-codes (A4000-A8999) are bundled into the related E/M or procedure under Medicare and pay separately only when billed by a DME supplier with a valid PTAN. Practice billing of A-codes typically results in CO-97 bundling denial.

Ambulance services (A0021-A0999) require origin/destination modifiers (RH for residence to hospital, etc.). Missing origin/destination modifier is the top denial reason for ambulance claims.
