E/M (Evaluation and Management)

CPT code family covering office visits, hospital visits, consultations, and other cognitive (non-procedural) physician work. Office E/M codes (99202-99205 for new, 99211-99215 for established) account for over 30 percent of physician spending. Since 2021, office E/M code selection is based on either Medical Decision Making (MDM) or Total Time on date of encounter. The most common error is undercoding 99213 when the chart supports 99214 (a $48 per visit revenue gap).
