Four-digit code on the UB-04 hospital claim describing where the service was performed or what type of service it was. 385 active codes. Examples: 0250 (pharmacy general), 0450 (emergency room), 0636 (drugs requiring detailed coding). Each line on a UB-04 needs a revenue code paired with a HCPCS code. Wrong revenue codes are a frequent reason for hospital outpatient denials.
