A premium is the monthly payment that a person or employer makes to maintain health insurance coverage. Premiums are paid regardless of whether the member uses any healthcare services that month. As a provider, you do not directly interact with premiums, but they affect your practice indirectly. Plans with higher premiums tend to have richer benefits: lower deductibles, lower copays, and broader networks. Plans with lower premiums tend to have higher deductibles, higher cost sharing, and narrower networks. The trend toward lower-premium, higher-deductible plans means providers are collecting more from patients directly and relying less on insurance payments for the initial portion of each patient's care each year. This shifts financial risk from the payer to the patient and, by extension, to you if the patient does not pay their share. Premium levels also affect network decisions by the payer. Lower-premium plans need to control costs more aggressively, which often means narrower networks, lower provider reimbursement, and more prior authorization requirements. When a payer offers you a contract with a fee schedule that seems low, it may be for their narrow-network, low-premium product. Understanding the premium landscape in your market helps you predict patient behavior and plan your revenue cycle strategy. If the dominant employer in your area switched from a PPO plan to a high-deductible plan, you can expect increased patient payment responsibility and should adjust your collection processes accordingly.
