NDC (National Drug Code)

Definition

FDA-assigned 10 or 11 digit identifier for human drugs and biologics, formatted as labeler-product-package (e.g., 0078-0658-15). Required on most J-code drug claims for accurate reimbursement. NDC must be reported with quantity in milliliters or units administered. Most payers reject drug claims missing the NDC even when the J-code is correct.
