Definition

NCCI Medically Unlikely Edits set the maximum units of a HCPCS/CPT code one provider can bill for one beneficiary on one date of service. 33,223 active MUEs in 2026. Adjudication types: Date of Service Edits (cannot bypass even with modifier), Date of Service Policy Edits (sometimes bypassable), Claim Line Edits (modifier-bypassable). Exceeding the MUE without a justifying modifier and documentation triggers automatic line denial.
