Your state medical license is the foundational credential that everything else depends on. Without an active, unrestricted license in the state where you practice, you cannot enroll with any payer, period. Every state has its own medical board that issues licenses and sets its own requirements for education, examination, and continuing medical education (CME). If you practice in multiple states, you need a separate license in each state. There is no national medical license, although the Interstate Medical Licensure Compact has made multi-state licensing easier for physicians who qualify. Credentialing applications require your license number, issue date, expiration date, and the licensing state. The payer verifies this information directly with the state medical board through primary source verification. They also check for any disciplinary actions, restrictions, probations, or previous license surrenders. License renewal requirements vary by state, but most require a certain number of CME hours (typically 25 to 50 hours per renewal cycle) and payment of a renewal fee. Some states also require specific coursework topics like opioid prescribing, pain management, or ethics. A lapsed license is one of the most common reasons credentialing applications get delayed or denied. Even a one-day gap in licensure can cause problems. If your license expires and you renew it, some payers want an explanation of the gap. Set your license renewal reminders well in advance, ideally 90 days before expiration. If you have ever had a license action in any state, you must disclose it on every credentialing application. This includes voluntary surrenders, reprimands, fines, probation, or any other disciplinary action. Failing to disclose is worse than the action itself.
