LCDs include specific covered ICD-10 codes, non-covered ICD-10 codes, documentation requirements, and frequency limits. Each LCD has an associated Local Coverage Article (LCA) that lists the actual code lists. Always verify LCD/LCA before billing diagnostic tests, DME, and outpatient procedures in your jurisdiction.
