Electronic Remittance Advice. The X12 transaction payers send back showing claim adjudication results: charges, allowed amount, paid amount, patient responsibility, and CARC/RARC denial codes. The 835 enables auto-posting to your practice management system. Each 835 ties back to one or more 837 claims through the original claim ID. Monitoring 835 patterns is the main way to identify denial trends.
