Five-digit code set maintained by the American Medical Association that describes medical, surgical, and diagnostic services. CPT 2026 contains 11,025 active codes plus modifiers. Categories: I (procedures, 99202-99499 + 0001A-0989F), II (performance measurement, 0001F-9999F), III (emerging technology, 0001T+). Most-billed CPTs nationally: 99214 (98.5M Medicare claims/year) and 99213 (73.6M). CPT codes are updated each January 1.
