Contractual Adjustment

The mandatory write-off equal to the difference between billed charges and the contracted allowed amount. Reported on the 835 with group code CO. Provider cannot bill the patient for the contractual amount. Failure to write off contractual amounts and instead billing the patient is a violation of most provider contracts and may also violate state insurance laws.
